“Circle of Hope” Award FAQ’s
Application Deadline
The Application period is open annually from March 1-September 30. A link to the application will be available during the enrollment period.
Award Date
The Putt Away Cancer “Circle of Hope” Award will be awarded December 1st.
What You Should Know
The Putt Away Cancer “Circle of Hope” award application can be found on this site. You will also find a list of documents needed to complete the application and be considered for the scholarship. Please ensure you have completed the application in its entirety prior to submission. All incomplete applications will be discarded.
Please ensure to leave valid contact information. There is a limited amount of scholarship awards available. All selected recipients will be notified via email or phone call regarding your application status around November 1st. Disbursements will be sent by December 1st.

Eligibility Requirements
- Must be a US resident.
- Must be a cancer patient/cancer survivor (documentation required. Must submit a consent form in order for us to receive information validating you are/were a cancer patient. Your application will not be considered without this form being signed).
- Must complete a disclosure statement allowing or declining the release of your name and state. However, Putt Away Cancer reserves the right to use the photo or video images and/or statements of scholarship recipients in any promotional materials.
